Assumption of Risk, Release, Media Consent and Conduct Agreement
Soccer and related tournament activities involve risks including collision, falls, physical contact, sprains, fractures, concussion, illness, heat exposure, weather conditions, serious injury, disability, death and other foreseeable or unforeseen circumstances. The team representative acknowledges these risks and confirms that each listed participant is voluntarily participating and is responsible for determining whether they are physically fit and medically able to play.
To the fullest extent permitted by law, the representative, for the team and only to the extent legally authorized by each participant, releases and holds harmless Himalaya Baltimore FC, its officers, directors, volunteers, referees, organizers, sponsors, venue owners and agents (“Released Parties”) from claims arising from ordinary negligence connected with tournament participation. This release does not apply where prohibited by law, including gross negligence or intentional misconduct.
The representative authorizes tournament personnel to seek reasonable emergency medical assistance when a participant cannot provide consent and acknowledges that the participant is responsible for related medical expenses. The Released Parties are not responsible for lost, stolen or damaged personal property. The representative also confirms that the tournament rules and conduct requirements have been shared with the full team.
